Add MCP coverage-state and provenance metadata to each fetched document list.
The wrapper stamps every document with mcpCoverageState/mcpProvenance so the manifest and downstream analysis can distinguish full-text, metadata, and empty-search conditions without reparsing the original MCP payload.
